Diseases & Conditions

Through lifestyle interventions, the UofL Health – Healthy Lifestyle Centers address chronic conditions, such as heart disease, diabetes, cancer, obesity and more. We support and help individuals develop wellness programs personalized to their own health goals.

 Our program options include:

  • Cardiac Rehabilitation
  • Pulmonary Rehabilitation
  • Supervised Exercise Therapy for Peripheral Artery Disease
  • Lifestyle Medicine

Treatments & Services

Our services begin with a clinical assessment of personal health and risk factors. From there, treatment programs are designed to fit an individual’s specific needs that can include:

  • Fitness training with treadmills, elliptical trainers, recumbent bicycles, and free weights.
  • Educational classes which address an array of risk factors to ensure individuals have the information, support, and resources needed for optimal recovery.
  • One-on-one consultation with a registered dietitian to evaluate and modify diet.
  • One-on-one consultation with a licensed clinical social worker to develop a plan to accomplish goals.

Cardiac Rehab and Qualifying Conditions

Cardiac rehab reduces the chances of a fatal heart attack by 20-30%. It can help lessen the effects of heart disease while improving stamina and ability to return to usual activities. Generally, the program occurs over three months, with visits three times per week.

Because our advanced cardiac rehab takes a holistic approach to heart health, our program stands above traditional options. This advanced program offers educational classes, dietary consultations, and stress management guidance, in addition to the fitness training and monitoring found in traditional cardiac rehab. Our cardiac rehab is covered by Medicare/Medicaid and commercial insurance for the following conditions:

  • Bypass surgery
  • Stent/Angioplasty
  • Valve surgery
  • Heart transplant
  • Heart attack
  • Heart failure (EF< 35%)
  • Stable angina

Pulmonary Rehab and Qualifying Conditions

Pulmonary rehab is designed for individuals with emphysema, chronic bronchitis, chronic asthma, or COPD. The program is designed to teach people with breathing problems how to decrease their shortness of breath with most activities.

Pulmonary rehab focuses on the individual by helping them improve their endurance levels through monitored exercise. The program gives individuals a better understanding of their condition and teaches them to manage it on a daily basis through energy conservation, breathing techniques, diet, and stress management. Pulmonary rehab also helps individuals regain self-confidence. Patients may notice they are stronger, and see a decrease in symptoms such as wheezing, coughing, and shortness of breath.

Generally, the program occurs over three months, meeting for 24-36 visits, twice per week. Medicare/Medicaid and commercial insurance will cover pulmonary rehab for these conditions:

  • COPD
  • Emphysema
  • Chronic bronchitis
  • Pulmonary fibrosis
  • Lung cancer and lung cancer surgery
  • Lung volume reduction surgery
  • Lung transplant

Supervised Exercise Therapy

The Healthy Lifestyle Centers offer a specially designed exercise program tailored to people with peripheral artery disease (PAD). PAD is a vascular disease, caused by a gradual build-up of plaque within the leg arteries. It may cause disabling symptoms, such as leg cramping while walking, leg pain while resting, nerve damage and/or skin breakdown. This exercise therapy helps those suffering from PAD pain by targeting the muscles that hurt when you walk, such as the calf, thigh and buttock.

Supervised exercise therapy helps improve circulation and available metabolites that can reduce or get rid of pain while walking. The program pairs moderate intensity exercise training with lifestyle changes. Most insurance plans cover supervised exercise therapy.

Lifestyle Medicine

When it comes to total body health and wellness, through a membership with the Lifestyle Medicine
