When will I be able to access my information in MyChart?
You may now request access to the UofL Physicians MyChart patient portal. However, your information may not show up until the provider has had a chance to review your information with you.

Is this confidential?
We’ve taken extra steps to ensure your health information remains confidential. Your records are safe from unauthorized access because our patient portals are password-protected and information is delivered via an encrypted connection.

- Patients 18 and older
- Parents of minor children
- Legal guardians (legal documentation is required)
- Emancipated minors (Patients younger than 18 who are legally married, living apart from parents, or those who have borne or fathered a child.)

When will the UofL Physicians FollowMyHealth patient portal go away?
Your historical data will not transition to MyChart; however, it will still be available in the FollowMyHealth™ patient portal. The ability to send messages to your UofL Physicians provider through FollowMyHealth will be turned off when MyChart launches for your practice (by March 3, 2021 for all practices).
If you wish to have your own copy of the information in UofL Physicians FollowMyHealth patient portal without needing to access a separate account, we recommend you print/email a copy of your medical records currently in the UofL Physicians FollowMyHealth patient portal. Instructions may be found below.
How do I print, email or fax a copy of my UofL Physicians FollowMyHealth records?
To print or email your records from FollowMyHealth, follow these steps:
- Login to your FollowMyHealth account at followmyhealth.com.
- Select the My Health tab at the top.
- Select the “send” button in the top, right corner. This will give you the option to email, print or fax your records.
- Select “print all” or “email all” to print or email your full records.
- When you print or email, this is an unencrypted message transmission. If you wish to send it encrypted, use the “export” option instead.
