Health Management Services, a UofL Physicians program, is making health happen. Whether you need help managing your diabetes, tips to better control your cholesterol, or desire to make better food choices, we can help! This program provides a number of exciting, specialized services focused on improving your overall well-being by better understanding and managing your health condition.

The program partners with local employers to offer health management services to the employers’ medical plan participants. Currently, this program is offered through University of Louisville’s Get Healthy Now wellness program. Goals of Health Management Services:

  • Support the health goals established by participants and their care team.
  • Coordinate care across a team of doctors and wellness professionals through a collaborative health management plan.
  • Expand access to providers, programs and community resources to improve care, outcomes and quality of life.
  • Empower participants to manage their health by improving their knowledge of conditions and self-care options.
  • Integrate our services with employers’ wellness programs and services to better serve our participants.

The first step is a brief lifestyle assessment so that we may better assist you with managing your health condition. This assessment allows us to take into account biological, social and psychological issues related to your condition.

Working as your health care partner, we will then develop an individualized program with realistic goals and expectations, taking into consideration your health, overall lifestyle, family support system and schedule demands.


Biopsychosocial support

Bio-psychosocial assessments are conducted to gain a better understanding of the whole person, the barriers that might be preventing them from achieving their health goals, and to help inform the health or wellness coach of appropriate interventions for each individual. Readiness to change is assessed and a diverse array of psychological and behavioral change tools to empower individuals to become better self- managers of their health is employed. 

Care Management

Care management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The program manager will help participants identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for both the participant and the reimbursement source.  The program manager will facilitate the assessment process, planning, and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes.

Disease-Specific Education

Disease-specific education classes are offered to participants experiencing the same condition.  It is an opportunity to meet together to discuss the daily challenges they face while trying to manage a chronic condition. The group format allows for participants to brainstorm and share ideas, and normalizes a patient's experience.  The classes instill a sense of hope and also help to build motivation for change. 

Educational Workshops

Educational workshops are a series of classes that focus on health education and literacy.  

These workshops are conducted through a team of Diabetes Educators, Dietitians, Health Coaches, Nurse Practitioners and other health professionals.  All workshops are designed to highlight the benefits of adopting a healthy lifestyle.

Medication Therapy Management

Medication Therapy Management  is a service that is delivered through the Know Your Rx Pharmacy Coalition.  It is designed to facilitate collaboration among the pharmacist, patient, physician, and other health care professionals to promote safe and effective medication use and achieve optimal patient outcomes.

Nutrition Counseling

Nutrition counseling is an ongoing process in which a health professional will work with you to assess your dietary intake and identify areas where change is needed. Nutrition counseling is an option for those who are interested in making and maintaining dietary changes to improve their health.  As a part of counseling, you may receive educational materials, support, and follow-up to assist you with your journey to better health. Weight Management

For our UofL participants, "Healthy U" is a program offered through the UofL Get Healthy Now program.  The program is a 12-week class that promotes healthy lifestyle changes through physical activity and improved nutrition.  The class is designed to give you an accurate understanding of ways to integrate optimal health behaviors into your everyday life.  Throughout the free, 12-week program, you’ll experience what a healthy weight means to you; learn how to make sustainable lifestyle changes; learn to lose weight safely, effectively and keep it off; receive a personalized work out plan; and earn incentives to keep you active and healthy.

Wellness Coaching

Through wellness coaching, participants will identify their wellness priorities while working one-on-one with a wellness coach.  A wellness coach will use behavior change tools to empower participants to take charge, connect with their motivators, and learn goal setting strategies.  Wellness coaching creates a powerful partnership between client and coach, provides support, guidance, and encouragement, is free of judgment and facilitates change by focusing on your past success and strengths. First step:

To participate in the Health Management Services program, simply email your name, preferred contact information and a brief summary of your health condition to HealthManagementServices@ulp.org. A UofL Physicians program manager will reach out to you with more information regarding step two.

Second step:

A lifestyle assessment will be completed in our office during program orientation.

Third step:

Your Health Management Services team will partner with you to establish and reach your individualized health goals, and assist in the management of diabetes, COPD, hypertension, hyperlipidemia or asthma. Who is eligible to participate?  

Health Management Services program which provides services to employees, spouses and eligible dependents who participate in the University of Louisville’s Anthem (or any subsequent health plan) who have at least one (1) of the chronic diagnosed conditions.

Which chronic conditions are included?  

Health Management Services supports members with the following diagnosed chronic conditions: 

  • Chronic Obstructive Pulmonary Disease (COPD)
  • Diabetes
  • High Blood Pressure (Hypertension) 
  • High Cholesterol (Hyperlipidemia)
  • Asthma

What can I expect from this program?  

The Health Management Services team will support you in medication management, educational classes, specialty referrals (when medically indicated), coordination and communication with your primary care provider, specialists, etc. The participant can expect to gain a fresh perspective on managing the condition, education, personal goal planning, etc.  

Do I have to switch providers if I participate in this program?  

No. This program is not designed to replace your current provider. In fact, we want to work with you and your provider to assist you in meeting your healthcare goals and offer additional resources to keep you healthy.

Will I receive the $0 copay pharmacy incentive related to all of my conditions?  

Medications for your primary condition will be eligible to receive the pharmacy incentive. Unassociated conditions will be subject to the standard pharmacy benefit copay.

What do I have to do to participate?  

Active engagement and participation is required to receive the program benefits. These requirements include:  

  • Participation in the initial intake process and appropriate assessment in order to develop an appropriate Individualized Care Plan set forth by the Health Management Services “care team”
  • Participation in, at the minimum, quarterly in-person visits and monthly follow-up calls with Health Management Services provider(s) and/or staff
  • Attendance at least two educational sessions per year
  • Notification to the care team of any transitions of care (hospitalization, Emergency Department visits, urgent care, skilled nursing facility etc.)
  • Attendance to any referral appointment coordinated by the Health Management Service team
  • Returning calls initiated by the Health Management Service team in a timely manner

What if I sign up for participation in the middle of the year?  

Participation requirements will be modified “prorated” based on program entry date (calendar year).

What happens if I am contacted but choose not to participate?  

Participation in the program is not mandatory; however, to obtain services as well as the pharmacy incentive, you must elect to participate. The health benefits of working toward managing your condition(s) are immeasurable. We hope to keep you in the "driver’s seat" – living every day with increased energy and vitality.

How do I participate in the Health Management Program?  

Simply call 502-588-0770 or email HealthManagementServices@ulp.org to schedule an intake appointment.

How is my health information protected?  

The program operates in accordance with medical privacy standards established by applicable federal and state laws. All information regarding your health condition will be completely confidential and will not be shared with your employer.

Who will have access to my Personal Health Information (PHI)?  

Only employees authorized to access your PHI are permitted to do so. The information accessed shall be the minimum necessary to perform one's assigned job duties.

Can I end my enrollment in the program at any time?  

Yes. You may “opt out” of program at any time. To do so, submit a request to terminate services to: Health Management Services, Attn: Program Enrollment at 601 Presidents Blvd, Louisville, KY 40217 or email HealthManagementServices@ulp.org.

Meet our Nurse Practitioner

Mary Corbett is the Advanced Practice Registered Nurse for the Health Management Services program. She holds a Bachelor of Science in Nursing and a Master of Science in Nursing, both from the University of Louisville.

Mary will provide UofL employees with the opportunity to receive compassionate, coordinated care in order to improve their overall quality of life. Her goal is "To understand the health goals of participants in order to help facilitate optimal health for them."

Mary's clinical interests include chronic disease management, health and wellness, and adult/geriatric quality of life care.

Program Manager’s Role

Program managers play an essential role in the coordination of your care upon entry into this specialized health management program. We work with participants to match their needs to appropriate services as they navigate through the health care system. These services may include social services, transportation services, and even medication reminders -- whatever may be needed in order to remove barriers to high quality care. As you enter into this program, orientation and assessments are a necessary part of creating patient-centered individual care plans. Following up with our program participants is also an integral part of ensuring that your health is being carefully managed. As you can see, the role of the program manager encompasses various aspects of assisting you throughout your wellness journey.  

Role of the Employer

UofL Physicians is working closely with UofL’s Get Healthy Now to ensure the seamless integration of services available to UofL employees and their dependents. The Get Healthy Now Wellness Center is a 22,000 square foot employee-based facility that offers comprehensive well-being support through health and wellness classes, coaching, motivational programs, state-of-the-art fitness equipment, integrated clinical care and friendly, professional staff. Whether participants are interested in educational, fitness or mindfulness classes, there is something for everyone!   

Partnering with Registered Dietitians

The Health Management Services program, as a part of the UofL Physicians network of providers, partners with a registered dietitian to assist participants with improving their personal diet and nutrition goals.
