In the past, you might have kept track of doctor’s appointments or health reminders using Post-it Notes, an electronic calendar, a paper notebook or appointment cards in your wallet or on the refrigerator. These are just some of the ways patients have kept track of their appointments, blood pressure readings, or upcoming immunizations for themselves or their family members in the past.

Today there is a better way that is immediately available to you wherever you go on a computer or your smartphone. It is called a patient portal. The patient portal is just a website where a personal copy of your electronic medical record is stored. Through the patient portal, patients can review upcoming appointments, prior doctors’ notes, lab reports, send prescription refill requests, and even send a secure message to a provider. UofL Physicians choose Follow My Health to provide their patient portal.

The UofL Physicians patient portal is free for you to use and the information on the portal belongs to you. You are free to print, forward or do whatever you want with the information on the portal. You authorize UofL Physicians to update your health information whenever there are changes to your electronic record at UofL Physicians. There’s even an app so you can use your cell phone to interact with your UofL Physicians provider or practice wherever you are. Regardless of where you receive care in the future whether at UofL Physicians or another provider, you will always have access to the information in the UofL Physicians patient portal.

So where do you begin? If you do not already have a patient portal account with UofL Physicians, please visit the UofL Physicians website and register for an account. Once your account is created, you can login and review upcoming appointments, your providers’ office notes, immunizations, lab results, request refills and even send messages to your providers. You can even add new health information into your Follow My Health record for your providers to review. Your providers will automatically update your record whenever there is a change to your record. If you chose to receive email and text alerts, your provider can use the portal to communicate with you. The portal is even green because it eliminates most paper mail.

As you can see, the UofL Physicians patient portal can be a convenient tool for you and your family as well as well as your providers and their staff. Using the portal is just one way to we can help make achieving your health care goals easier.
